SMS Text Messaging

Crown Medical Practice will be operating a FREE text messaging service to remind patients of their upcoming appointments, invite them to make appointments and for any other matters relating to your care.  This system will only be available to patients over 12 years of age.

Having your most up to date mobile number recorded with us is essential and it is your responsibility to change/update it if it is no longer in use. Please update your details on our website using the Update your Details widget on the right hand side (Yellow widget). 

Your mobile phone number will only be used by the Practice and will not be passed to any other parties.

SMS Text Message Opt-Out

Please complete the fields on the form below and we will remove your mobile number from our SMS text messaging service. This will not affect your appointments or care; it just means you won’t receive any text messages from us to remind you of or invite you to make appointments or for any other matters relating to your care. We will still use your mobile number by calling you if you have supplied your number to us as part of your contact details. For your peace of mind, your details are encrypted and sent securely to us. It is important to complete all mandatory fields to allow us to identify you.
