Online Sexual Health Screening Tool

We now offer free, confidential, online sexual health screening for all our patients at Crown Medical Practice.

You will not be asked to provide any identifiable details. At the end of the questionnaire you will be given two unique numbers which you will need when you contact the practice to request the recommended screening tests.

Please note – the sexual health screening tool can only be used if you have no symptoms. If you have any symptoms like a change in bleeding, pain, urinary symptoms, discharge or a rash please make a GP appointment for review instead of using this screening tool by calling our reception team in 01463 214450.
